We accept payment in the form of cash, check, debit, credit card (Visa, MasterCard, Discover, or America Express), Care Credit and Lending Club. All payments for any dental service will be paid for before or at the time of service.

The cost of your treatment is determined based on your examination findings and treatment needs. Diagnosis and treatment recommendations are not made based on insurance coverage. Cost may increase in the event the doctor determines that your specific situation requires a more in-depth examination, and additional or special procedures.

Services deemed to be billable to your dental and/or medical insurance by Lucca Oral and Facial Surgery, will be submitted directly to a dental insurance carrier: An estimate of the amount covered by your insurance company will be provided at the time of your treatment, based on the best information provided to us at the time by you and the insurance carrier. Estimates may vary based on specific contract clauses you have with your carrier and should benefits be exhausted after your initial estimate and prior to treatment. The estimate is never a guarantee of benefits or the amount that will be paid.

Remember, we are not your insurance carrier. We are not responsible for their decision to cover or not cover a service. We will file insurance claims as a courtesy to our patients but this does not, transfer the responsibility of your financial obligation to the insurance company or to us. If the amount paid by the insurance company is less than or greater than the estimate, then you will be either billed the difference or issued a credit on the account in the event of an overpayment.

Know your insurance! Many patients aren’t familiar with the specifics of their dental insurance benefits. Your dental benefits are based on the agreement you or your employer negotiated with your insurance carrier. Please review the insurance benefit booklet provided by your employer to better understand the benefits they make available as part of your insurance coverage. The patient payment portion covered for procedures varies depending on the coverage provided by your employer.

You are ultimately responsible for payment. Acceptance of insurance assignment by our office does not absolve you of financial responsibility for services rendered. In the event your insurance carrier informs us that a service is not covered, partially covered, etc., you are responsible for the payment in full. We can make no guarantee of coverage or payment. You are responsible for all co-payments and deductibles before or at the time of your visit.

If you have concerns regarding your coverage, contact your insurance carrier directly. If you have an insurance carrier that we are not a contracted provider for, you are responsible for payment. Claims may be submitted for out-of-network benefits for open networks; however, we cannot determine your coverage or reimbursement rate. Contact your insurance carrier for out-of-network policy and coverage.

All patients must present photo identification at the initial visit to be kept on file. Failure to present photo ID will result in cancellation and rescheduling of your appointment.
It is extremely important for the quality of your care and safety that open and clear communication with your referring doctor be maintained to ensure medical errors are avoided. We require a written referral provided by your dentist or dental specialist at the time of consultation and/or treatment.
We require that a parent or legal guardian accompany all minors at the time of consultation and treatment.
If you are having a procedure under general anesthesia or conscious sedation (IV Sedation), a responsible and able- bodied adult (over the age of 18) must accompany you to the office and remain in the office waiting room during your procedure. Your escort will be called into the recovery room to be with you and to listen and assist with post-operative instructions along side you. This person is responsible to safely escort you home after your discharge from the office.

No anesthesia-assisted procedure will begin without the presence of your escort at the time of check in. Failure to have an escort present will result in appointment cancellation and rescheduling. In addition, it is extremely important for your safety that you have nothing to eat or drink for a full 8 hours prior to the time of your scheduled appointment. Failure to comply with the fasting requirements for anesthesia will also result in cancellation and rescheduling of you procedure.

The use of all recording devices (including cell phones for photo, video and/or audio) in treatment and recovery areas is prohibited.

Your appointment is reserved specifically for you. A missed appointment affects the practice due to the loss of revenue from an empty appointment. A missed appointment impacts the access of other patients to more timely care. We will make all efforts to reschedule any appointment to meet your needs if given at least 24- hour notice (Of course, we actually encourage you to cancel your appointment if you are ill, and obvious conditions apply).

We ask that our patients arrive on time for their designated appointment. Arriving more than 15 minutes past your scheduled appointment time will result in a failed appointment. If you are having anesthesia or IV sedation, your escort must be present with you at the time of check in. No new appointments will be made unless your account balance is paid in full. Anyone who does not show for 3 appointments will be dismissed from the practice.
