Accepted Forms of Payment
We accept payment in the form of cash, check, debit card, credit card (Visa, MasterCard, Discover, or America Express), and CareCredit. All payments for any dental service will be paid for before or at the time of service.
Insurance and Payments
Lucca Oral and Facial Surgery is a non-contracted (out-of-network, non-participating) provider for all medical and dental benefit plans. The cost of your treatment is determined based on your examination findings and treatment needs. Payment for services to be rendered by Lucca Oral and Facial Surgery are due in full before or at the time of service.
Services billable to your dental and/or medical insurance by Lucca Oral and Facial Surgery, will be submitted directly to your dental benefit carrier (and medical insurance only when applicable). As a courtesy, we will submit claims and file any necessary supplementary material on your behalf – to help maximize any benefits available to you from your dental benefit plan and/or insurance carrier (s). Any benefits payable will be paid to you directly by your dental benefit or insurance provider. Claims paid for treatment at Lucca Oral and Facial Surgery will be paid according to your dental benefit or insurance provider’s non-contracted rate for covered services.
We encourage you to familiarize yourself with your insurance benefits. We understand that many patients are not familiar with the specifics of their dental and medical insurance benefits. Your benefits are typically determined based on an agreement you or your employer negotiated with your insurance carrier. If you have any concerns regarding your coverage, we encourage you to call your insurance or benefit provider directly and/or speak to your Human Resources department to review and understand what is available as part of your insurance coverage with non-contracted providers. It is your responsibility to be familiar with your insurance coverage prior to treatment. Benefits payable for procedures vary depending on the coverage provided by your plan and/or negotiated by your employer.
Every reasonable attempt to successfully submit claims on your behalf for out-of-network benefits in open networks will be made. We are not responsible for your plan’s decision to cover or not cover a service and we can make no guarantee of coverage or payment from a third party. We are not responsible for lost or misplaced claims by third parties that result in delayed claims processing or claim denial. You are responsible for all payments due before or at the time of your visit.

Refunds
Certain procedures require an advanced order of materials, implant components, or may incur laboratory fees. These materials and fees are often unable to be returned and considered wasted. If your procedure requires that such an order be placed and fees incurred, a deposit for materials may be collected at the time your appointment is scheduled. Additionally, procedure appointments are scheduled as reserved blocks of time relative to the scope of your appointment. Lucca Oral and Facial Surgery will collect a deposit to reserve all procedure appointments. We require at least 48 hours’ notice for appointment cancellations. Failure to show for the appointment will result in the forfeiture of all or a part of the down payment in order to compensate for the loss of time and/or fees incurred.
Deposits and Cancelled Appointments
Any payments that are assigned to Lucca Oral and Facial Surgery by your dental plan or your insurance company that result in a credit balance on your account will be returned to you by check or refunded to your credit card pending your preference. When all claims have been coordinated and paid and the account has been closed -a refund check will be processed by the bank and sent to you at the address you have provided on file with us. Checks are generally processed in bulk on the first day of every month but may be processed sooner or refunded to your credit card on request.
(Note: Please inform us if you change your address and remember to institute mail forwarding. Reimbursement checks will be mailed to the last address we have on record and check cancellation fees may apply for re-issued checks).
